After completing level 4 but before advancing to the next level, go to coordinates 8,24 to find a group of boxes. Enter the small room found nearby, which has a pair of adjacent dumbwaiters. Block the opening of one of the dumbwaiters, then enter the other one. You will enter a hidden area which leads to a transportation booth that will teleport you to level 5.
